Internal medicine (Nephrology, Rheumatology, Immunology, Pulmonology), Oto-rhino-laryngology, Ophthalmology

Diseases

Systemic diseases, especially primary vasculitides, e.g. Wegener's granulomatosis, microscopic polyangiitis, panarteritis nodosa, Churg-Strauss-Syndrome

Specificity

98 % for Wegener's granulomatosis

Sensitivity

40 - 93 % depending on disease activity and extent
*Nölle et al., Ann. Intern. Med. 111 (1989) 28-40*

Characteristics

very specific for Wegener's granulomatosis due to the use of highly purified proteinase 3, the target antigen of C-ANCA.
Due to impure antigen-preparations (granule extracts) used by other manufacturers, several false-positive results can be obtained with their ELISAs, e.g. in patients with other autoimmune disorders like SLE, Goodpasture's syndrome or Henoch-Schoenlein purpura. Moreover, some patients with high concentrations of myeloperoxidase antibodies can be found positive in these ELISAs *Rasmussen et al., J. Immunol. Methods 712 (1990) 139-145*
  * no subjective criteria as in indirect immunofluorescence
  * follow-up control of disease activity by quantitative results
  * parallel processing with the Anti-MPO-ELISA for detection of myeloperoxidase autoantibodies is possible; many of the reagents can be used from either of the kits
